Year 5 & 6 Mobile Phone Permission

My child will be bringing their mobile phone in to school. I acknowledge that my child's phone will be locked away during the school day and returned to my child at the end of the day.

Year 5 & 6 Permission for my child to walk home on their own

From Year 5 Great Denham Primary allows children to walk home alone after school with their parents/carers permission. Please complete the form indicating what days you allow your child to walk home.

Year 4 Request for my child to walk home on their own

In exceptional circumstances, the school may allow children in Year 4 to walk home after school. Mrs Burgess will look at each case individually and be in touch as to whether this will be authorised.

Application for Leave of Absence

As a Parent/Carer, you should fill in this form if you want to take your child out of school for any reason during term time. IMPORTANT INFORMATION FOR PARENTS/CARERS * In line with Government Legislation with effect from 1st September 2013 Headteachers' have the discretion to grant leave, but will only do so in exceptional circumstances. If the Headteacher grants a leave request it will be for them to determine the length of time that child can be away from school. This leave is unlikely, however, to be granted for the purpose of a family holiday. * Parents can be fined for taking their child on holiday during term time without consent from the school.

iPad Query Form

Please give as much information as possible regarding the issue you are experiencing with your child's iPad. Your child should keep their iPad with them and a member of the IT support team will collect it from them when they are available to process the query.

Permission to go on Local Walks

From time to time we will take the children out into the local community on walks and visits. Rather than having to do a permission slip every time, we would like to ask your permission to take your child out of school and visit the local area. We will always inform you in the weekly newsletter if we plan to take the children out and what the purpose of the visit is. Please note that a local walk will be carefully planned to ensure the appropriate adult ratios are in place as we would on any school trip. We will not be able to take any children out who do not have parental permission.

e-Safety Rules & Permission

Parent's Consent for Internet Access By completing this form I confirm I have read and understood the school e-Safety Rules and give permission for my son/daughter to access the Internet (please see separate e-Safety Rules). I understand that the school will take all reasonable precautions to ensure that pupils cannot access inappropriate materials but I appreciate that this is a difficult task. I understand that the school cannot be held responsible for the content of materials accessed through the Internet. I agree that the school is not liable for any damages arising from use of the Internet facilities.

Permission for photos

Occasionally we may take photographs of the children at our school. We may use these images in our school prospectus or in other printed publications that we produce, as well as on our website. We may also make video or webcam recordings for school-to-school conferences, monitoring or other educational use. From time to time, our school may be visited by the media who will take photographs or film footage of a visiting dignitary or other high profile event. Pupils will often appear in these images, which may appear in local or national newspapers, or on televised news programmes. Due to GDPR compliance all Parents need to reply yes/no to the following questions, if we do not receive your form back your child will not be allowed to take part in any photographs. I agree that my son/daughter's work may be electronically published. I also agree that appropriate images and video that include my son/daughter may be published subject to the school rule that photographs will not be accompanied by pupil names.
